Asthénie progressive au décours d'une chirurgie sous anesthésie générale

ATCD

• 08/1990 : exérèse d'un thymome
• 12/1993 : récidive ?
• 06/1994 : sternotomie exploratrice, chimiothérapie + radiothérapie
• 06/1998 : 2ème récidive avec métastases pleurales => exérèse en 09/1998

HISTOIRE DE LA MALADIE

1ère hospitalisation :

• Dyspnée +++
• Déficit moteur à l'effort d'un hémicorps
• Chutes :
  • EMG normal
  • Test au TENSILON : positif
  • Anticorps anti-récepteur à l'Acétylcholine (Ac anti-Rc Ach) : en attente

2ème hospitalisation :

• Ptosis bilatéral
• Fatigabilité à l'effort
• Mastication altérée :
  • EMG positif, bloc neuromusculaire post-synaptique
  • Ac anti-Rc Ach : positif
  • Amélioration clinique sous MESTINON

DISCUSSION

Intérêt de cette observation :

Apparition d'une myasthénie 8 ans après le diagnostic d'un thymome malin, chez un patient qui aurait subi plusieurs anesthésies sans problèmes
